Please note that the following completed forms are needed to register for youth camps:

- Registration Form (along with $50.00 deposit)
- Health History Record (double-sided)
- Copy of Insurance Card (front & back)
- Permission Statements
- Camper Release
- Challenge Course Release (double-sided)

Please note that the following completed forms are needed to register for family camps:

- Registration Form for Each Family Member (along with a family deposit of $50.00)
- Health History Record for Each Family Member (double-sided)
